Birth Certificate Requirements
A birth certificate is an essential document every citizen must hold as legal proof of birth registration. Requirements include a birth attestation letter from a doctor, midwife, or birth attendant — or a Sworn Statement of Absolute Responsibility (SPTJM) for birth data truthfulness if no letter exists.
Also prepare the parents' marriage certificate or marriage excerpt (or an SPTJM affirming them as a married couple), the family card where the child will be registered, and the e-ID of a parent, guardian, or reporter. The good news: processing a birth certificate does not require a cover letter from the RT/RW neighborhood unit.
How to Apply Online
Most regions now serve birth certificate applications online through each district/city's official Dukcapil app or website. Make sure the app you download is officially issued by the respective local government to avoid scams.
The flow: register an account using your national ID number, fill in the civil-record document service form (F-2.01), then upload the requirement files in clearly legible JPG or PDF format. Applicants receive an application receipt, after which officers verify and validate the data against the SIAK database.
If approved, the civil registry official issues a registration number and signs the certificate electronically. Notification is sent by email, and the birth certificate is issued as an electronically signed (TTE) PDF complete with a QR code as an authenticity marker. In addition to the birth certificate, the same online application usually also processes the family card update, so the family's civil data is synchronized at once.
Self-Printing and the Offline Option
The digital birth certificate can be self-printed at home on plain white 80-gram A4 HVS paper. Although self-printed, the document carries the same legal force as the office-printed version, thanks to the QR code at the bottom-right corner replacing the wet signature and stamp. Keep the birth certificate PDF in several safe places, such as your personal email and cloud storage, so it is easy to access again whenever needed.
For the offline route, visit the nearest civil registry office, take a queue number, fill in the form, then hand the form and requirement files to the counter officer. All Dukcapil civil administration services are free — never pay anyone.
